01. Be a Club Doers Member in good standing

02. Present a Club Doers Membership Card PassCode ID# on the order Form

03. Present a current credit card or acceptable form of payment and matching proof of your current physical delivery address.

04. Have the same current address on both their credit Card and invoice to open an account

05. Pay a $29.00 refundable cassette insurance & damage fee that is  returned to members in good standing.

Educational and information audio cassettes may be borrowed up to two weeks in advance. All other audio cassettes are available on a walk- in basis only and may not be reserved.             (THE AVAILABILITY OF AUDIO CASSETTES WILL NOT BE LOOKED UP FOR MEMBERS DUE TO LACK OF STAFF, SO PLEASE DON'T ASK WHEN A SPECIFIC TITLE WILL BE AVAILABLE, ETC.) AUDIO CASSETTES ARE STRICTLY A FIRST COME, FIRST SERVED ITEM.

A maximum of 6 units may be borrowed at any one time for a loan period of two weeks. Cassettes must be returned no later than the date they are due.

All cassettes checked out by a member with the same due date must be returned at the same time (no partial returns) and cassettes may not be renewed with a consecutive periods.

Audio cassettes are to be returned using the U.S. Postal Service Priority 2-3 Day mail or UPS and may not be returned in person. There will be a $1.00 charge per unit that is not rewound to the beginning.

THE COPYRIGHT PROPRIETOR HAS LICENSED THE INFORMATION CONTAINED IN THESE AUDIO CASSETTES FOR PRIVATE HOME USE ONLY AND PROHIBITS ANY OTHER USE, COPYING, LOANING, REPRODUCTION OR PERFORMANCE IN PUBLIC IN WHOLE OR IN PART.

All Club Doers Membership Card Numbers must be presented each time cassettes are checked out. You are not allowed to check out cassettes on someone else's card.

The person in whose name cassettes are checked out assumes complete financial responsibility for the cassettes. The insurance fee will provide coverage for up to 80% of the cost of a damaged audio cassette. The member is responsible for the remaining 20% of the cost. Rewind cassettes after playing. There will be a $1.00 charge per unit not rewound.

Do not attempt to repair any cassette damage. Be sure to report any damaged tapes to the Audio Staff.

The Club Doers Library is not responsible for damage to members' cassette players.

"I understand and agree that the SEEDS OF SUCCESS CLUB DOERS BOSSA abides by the Federal Laws that prohibits, reproduction of copyrighted audio and video cassettes. As a member, I shall also uphold the rights of the author by not making or permitting copies to be made of CLUB DOERS materials. I agree not to loan or play CLUB DOERS materials to anyone for any reason.

I will inspect requested materials upon receipt and notify CLUB DOERS within 24 hours in the event any materials are damaged or in any way incomplete, so as not to be held responsible for the condition in which they were delivered. In the event they are lost or damaged while in my possession, I agree to pay an amount equal to their retail value, printed in the current catalog and price list.

I shall have the materials mailed or ready for pick-up within two weeks after my receipt of same, unless I have notified CLUB DOERS at least three (3) days prior to the two-week deadline. A late fee of $1.00 per unit shall be due for each additional day or portion thereof that I retain the materials past the two weeks allotted.

Programs returned incomplete or damaged are considered outstanding until missing and/or damaged components are replaced or CLUB DOERS is compensated for same. Likewise, in the event I do not return my selection within four (4) weeks from the date on which it was delivered, I shall remit an amount equal to its retail value plus the total of accumulated late fees.

I understand that any future requests to preview programs will be subject to the conditions of the check-out agreement by which they are selected. I will maintain a library insurance deposit on account as a member in good standing. The insurance fee will provide coverage for a percentage of the cost of a damaged audio or video cassette. When necessary I will be responsible for the cost of the remaining percentage. I will rewind all cassettes or pay the $1.00 labor charge per unit and report any damaged materials.

It is my understanding that the SEEDS OF SUCCESS CLUB DOERS can not and does not assume responsibility for the accuracy of the materials, and all representations and warranties with respect thereto are hereby expressly denied. Therefore, I shall hold THE CLUB DOERS BOSSA harmless for any-direct or consequential damages arising from or in connection with the use of techniques or instructions given therein.

I have read and agree to the terms, and know that violation of this agreement may result in the revocation of my privilege to preview training materials from THE SEEDS OF SUCCESS CLUB DOERS BOSSA LEARNING LIBRARY. Should legal action be necessary to enforce the terms of this agreement, the prevailing party shall be entitled to legal fees."
